Pre-treatment Consultations

We ask all customers to complete a health consultation form, prior to any treatment taking place.

This will allow us, as health care professionals to provide you with the right level of care and service.

The information that you will be asked to provide will be strictly confidential and only used by us, for the purpose of evaluating your correct massage therapy.

This form is also a legal requirement and to comply with our business insurance as massage professionals.

Unfortunately, we are unable to provide any massage service without this form being completed first.
